Thanks to Carolina challenge, I finally take the step to create it for her.
As I do not have the punch/ dies to create the holes, thus, I manage to search online for making it using normal hole punch. Hope it fit to the memorydex tray/holder.

Below is the card that I had made for her. I like to create scene in my card making.
For the 2nd card, as I want to create a memory for her to congrat on the 6K milestone, I specially draw and color a Youtube Logo on the badge. Hope she will like it.
